Additional STAR Registry Benefits
Based on feedback from previous STAR users, STAR is perceived as a useful recruiting method for:

Request Forms
- Complete the brief Subject Request Form, providing basic information about the study and include a copy of the IRB approval letter and stamped Consent Form.
- Review the STAR Registry Health Questionnaire in order to list all fields to be used for inclusion or exclusion criteria (including gender, age, medical conditions etc.).
- Principal investigator should review and sign the User Agreement Form
If requesting to use STAR as a secondary data set:
- Complete the brief Data Set Request Form, providing basic information about the study and include a copy of the IRB approval letter and stamped Consent Form. Include the years for which longitudinal data is requested.
- Principal investigator should review and sign the Data Set User Agreement Form
